Step 1: Choose Your Service Plan
Select the service plan below that aligns with your goals and needs. This will redirect you to your first step.
*Note that Individual Labs and the Stress Management Video Series do not require a consent form and will redirect you to the payment pathway.
Step 2: Fill Out Your Consent Form
After selecting your desired service plan, you’ll be directed to a consent form specific to that plan. Please provide payment as well as accurate and complete information to proceed. After submitting your consent form, you should receive an email with a link directing you to your next step.
Step 3: Complete the Onboarding Form
Once you’ve submitted your consent form, you’ll receive an email with a link to the New Patient Onboarding Form. This form gathers essential details through our HIPAA compliant system and allows us to tailor our services to your needs.
Step 4: Schedule Your Appointment
Following the submission of your onboarding form, you’ll receive an email with a link to schedule your appointment. Choose a time that suits you best. Additionally, feel free to include any family members or support caregivers in the visit at your discretion.
